REGISTRATION

The following outlines the registration fees applicable by category:

Early Bird (until Sept. 17th, 2004): $285.00

Regular (after Sept. 17th, 2004): $385.00

You can register by fax in the following manner:

• Download the Registration Form, print it, complete it and submit it by fax to our Conference Organizers, Golden Planners Inc, at 613-565-2173. Please click here to download the Registration Form in pdf file format.

Registration fees include access to all conference sessions (Friday, November 12th – Sunday, November 14th), two continental breakfasts and two lunches (November 13 & 14), refreshment breaks and the reception on Friday, November 12th.

To be eligible for the discounted Early Bird registration fees, payment must be received by September 17th, 2004.

Registration Cancellations and Refunds: A full refund less a $50.00 administration fee will be processed upon receipt of written cancellation notice to Golden Planners Inc. postmarked or received by fax or email no later than October 12th, 2004. No refunds will be issued for cancellations received after October 12th, 2004. Substitutions from the same organization will be accepted if submitted in writing up to and including November 5th, 2004.
